The 0-67 Meltdown Incident refers to the nuclear meltdown of the 0-67 Starlight Nuclear Fusion Power Plant in Sigil's Central (a city on Planet Puppy), which occurred in the afternoon hours of June 7, 2017. The incident occurred during an inspection of the control rods belonging to the power plant's reactors when reactors 0, 1 and 4 began to melt down. The emergency shutdown system had failed in the process, which lead to the destruction of the power plant in a cataclysmic explosion and reducing the plant to a large, radioactive crater.
 
Shutdown failed, so an evacuation notice was put over the automated siren, but by that point 0 and 1 had melted, and shortly thereafter, 4.
